Pausing & restarting, the official way

Almost everyone on a GLP-1 pauses at some point — a surgery, a pregnancy, a coverage gap. The guidance for each is published; here's what it actually says, and what the label requires when you come back.

Quick answer: pauses are normal and planned for in the official guidance. The one rule that surprises people: after missing 2 or more weekly doses, you restart at a lower dose and build back up — never straight back to your old dose.

Before surgery or a procedure with anesthesia

In 2023, the American Society of Anesthesiologists suggested holding GLP-1s before elective procedures — a week ahead for weekly injections — because slowed stomach emptying raises aspiration risk under anesthesia. In October 2024, updated multi-society guidance shifted the default: most patients can continue their GLP-1, with the highest-risk patients following a 24-hour clear-liquid diet before the procedure instead. Because the guidance moved recently, surgical teams differ in what they ask for.

Next step: Tell your surgical and anesthesia team you take a GLP-1 as soon as the procedure is scheduled, and follow their instruction — they know your risk profile and their protocol.

Pregnancy — planned or discovered

The Wegovy prescribing information is unambiguous: discontinue at least 2 months before a planned pregnancy, because semaglutide stays in the body a long time, and discontinue immediately when a pregnancy is recognized. Weight loss offers no potential benefit during pregnancy and may harm the fetus, per the label.

Restarting: what the label says

These rules are from the Wegovy prescribing information's dosing section. Other GLP-1 labels have their own missed-dose rules — the pattern is similar, but confirm yours with your prescriber or pharmacist.

Missed one dose, less than 5 days ago

Take it as soon as possible, then return to your regular weekly day.

Missed one dose, 5 or more days ago

Skip it entirely and take the next dose on your regular day. Doubling up isn't a catch-up — it's an overdose risk.

Missed 2 or more consecutive doses

Don't resume at your old dose. The label says to restart at a lower dose and re-escalate, because gastrointestinal tolerance fades during a gap. Your prescriber picks the restarting dose.
